How do I place my order?

• Before you can add an Alzheimer’s Mirror™ test kit to your shopping cart, you must complete a short readiness quiz online to determine if you are an appropriate candidate for Alzheimer’s Mirror. If your results indicate you are appropriate for purchasing Alzheimer’s Mirror™, you will be able to complete the checkout process.

Why can’t I place an order by phone, fax or mail?

What is the Readiness Quiz?

• The Readiness Quiz is a questionnaire designed to determine if you are an appropriate candidate for the Alzheimer’s Mirror™ risk assessment. Alzheimer’s Mirror™ is intended for those who are concerned about their risk for developing Alzheimer’s disease. .
  
  This test is not intended for:
• Anyone under the age of 18
• Anyone who has diagnosed or suspected Alzheimer’s disease or dementia.
• Individuals who may have trouble coping emotionally with their results

How do I stop receiving emails and newsletters from Alzheimer’s Mirror™?

All of our emails contain an “unsubscribe” link at the bottom of the email. You may unsubscribe by clicking that link.

How do I delete my Alzheimer’s Mirror™ account?

• At any point, if you wish to end your relationship with Alzheimer’s Mirror™, please contact us using the customer contact form. Once we receive the request to end your account, we will delete all of your Personally Identifiable Information from our database and we will inactivate your account. You will receive a confirmation email once the deactivation is completed.

How do I delete all of my genetic data from Alzheimer’s Mirror™?

• If you decide to end your account with Alzheimer’s Mirror™, prior to receiving your results or after your risk disclosure, please send an email to: customercare@ALZmirror.com We will delete all of the Personally Identifiable Information linked to your genetic results immediately.
