Prior to using any materials, products, services, or websites of Alzheimer’s Mirror™, its parent or its subsidiaries, you, the user, hereby acknowledge that you have read, understand, and completely accept the following Terms and Conditions Throughout this Agreement, ‘Alzheimer’s Mirror™’ shall refer to itself, Smart Genetics, LLC and any parent or subsidiary.

Any use of Alzheimer’s Mirror™, its products, software, services and site, are subject to these terms and condition.

Use of our products, materials and services are hereby strictly prohibited should you, the user, not accept these Terms and Conditions.

By purchasing your Alzheimer’s Mirror™ assessment, you have taken a great step forward towards understanding your body and how genetics factor into your predisposition for Alzheimer’s disease

Description of Service

The Alzheimer’s Mirror™ genetic risk assessment service includes:

• Pre-test education and screening
• An Alzheimer’s disease risk assessment based on APOE genotype that involves saliva collection
• Education on additional risk factors including family history and non-genetic risk factors such as age, gender and ethnicity
• Disclosure and review with a certified genetic counselor combined with education and support to help you and your physicians plan for your future and for making health related lifestyle decisions

New or future products and services not described here are still bound by these TERMS AND CONDITIONS.

By accepting these Terms and Conditions, you acknowledge that Alzheimer’s Mirror™ may make changes or additions without prior notice to you. If you would like to keep up-to-date with our current Terms and Conditions, please check back with us prior to using any service or material. It is your choice to use our services.

Description of what our services are not

Alzheimer Mirror™ services are for informational and personal educational purposes only. The content found on the Alzheimer’s Mirror™ site or through our services is not, and is not meant to be, a substitute for professional healthcare advice. Interactions you may have with our genetic counselors are not, and are not meant to be a replacement for your doctor-patient relationship. The results of your risk assessment are NOT intended to be used as a diagnostic tool for Alzheimer’s disease.

Customer Conduct

Property Rights of Alzheimer’s Mirror™

You agree that Alzheimer’s Mirror™ owns all legal rights, titles and interests with regards to our product and services. Any information posted on the Alzheimer’s Mirror™ website may be confidential. You agree not to disclose any of this information to a third party without prior written consent from Alzheimer’s Mirror™.

You also agree that our content and services are protected by US and international copyright, trademarks and other rights and laws.

You are permitted to print one or more copies of your Genetic Risk Assessment results and a single copy of any other content on our site, for your noncommercial personal use only. You agree to not remove or alter any property right notices attached to this content.

You also agree not to use any trademark affiliated with Alzheimer’s Mirror™ for your personal use, or in a way that could cause confusion as to the owner of the trademark.

Your Saliva Sample

Regarding storage of your saliva sample, we follow the policy of our CLIA certified lab. Following extraction of the DNA from your sample collection, we ensure that our lab either destroys your sample or that all identifying information is removed from your sample.

The genetic results we obtain from collection of your sample are your property. We retain the rights that have been established in this consent form and with any additional Terms and Conditions.

Disclaimer

You understand and agree that

• Your use of Alzheimer’s Mirror™ and our services is your choice. You have the option of discontinuing the services at any time
• Alzheimer’s Mirror™ does not make any claims that our services will meet all of your expectations, or that our services will always go uninterrupted or without error
• Information downloaded from Alzheimer’s Mirror™ is through your own discretion and risk
• You will exercise caution when providing our company with any personally identifiable information about your or your family
• Alzheimer’s Mirror™ does not endorse any actions resulting from your choice of participating in our services.
• You will not hold Alzheimer’s Mirror™, Smart Genetics, LLC or any of our affiliates liable with regards to any actions that occur from your choice to use our product and services
• Alzheimer’s Mirror™ and Smart Genetics, LLC hereby disclaim any and warrantees of merchantability, fitness for a particular purpose and any and all other consumer warrantees under the Uniform Commercial Code or the adopted equivalent in your state.

• any personal action(s) you take after receiving results from our services
• your failure to keep your username or password protected
